As part of the government’s goal of fixing the front door of the NHS, GP surgeries must offer unlimited online access to patients. To better meet this demand, the surgery is using a “total triage” model for booking appointments via a system called patchs.

Book An Appointment
Most of our appointments and requests will be processed via patches. Logging in and completing the form yourself is the fastest way to get in touch. Our receptionists are available to help those who are having difficulty or are not digitally connected. Please access the patchs support page for help accessing the new system.
Regular Appointments – Online Questionnaire
Patients will be directed to complete an online health questionnaire which will then be reviewed by the duty clinician. They will triage your request and you will be signposted to the most relevant service or person. The online questionnaire should be used for:
- Acute or new health problems
- Ongoing health concerns (excluding annual reviews)
- Administrative queries (excluding regular prescription requests)
Our reception staff are unable to book appointments that have not been triaged. They can assist patients who are having difficulty completing the online health questionnaire or who are not digitally connected.
Long Term Health Conditions – Annual Reviews
We will continue to recall our patients who have long term health conditions or need regular check-ups based on your month of birth. Please collect your blood test form from the surgery and arrange a blood test when we recall you or during your birthday month.
All patients are required to either book online via UHNM – Patient Connect or patients can call the call centre: 01782 674242
You will be able to make an appointment with our practice nurse or in one of our long term health condition clinics over the phone with one of our receptionists, including:
- Annual reviews (Diabetes, Hypertension,
- Vaccinations (We are currently unable to to offer travel vaccinations)
Other Clinics and Services
We are fortunate to have a number of external services that run clinics from our site for the benefit of local patients. While they use our facilities, they are external to us and we do not have access to their clinical or admin systems. If you need to book, cancel, or speak to someone about these services, please contact them.

Cancelling Appointments
It is the patient’s responsibility to ensure they set reminders for and attend their appointments. When patients fail to attend pre-booked appointments it prevents someone else being seen.
We understand that circumstances can change and you may sometimes need to cancel or rearrange appointments. If you need to cancel an appointment, you can do this via the NHS App or by calling us. We only accept appointment cancellation requests made more than one hour in advance. If you exceed this timeframe, we will mark you as a DNA (Did Not Attend).
If your appointment is with a service that uses our consulting rooms, you will need to contact them directly to rearrange your appointment.
